My Funeral Wishes Form

We have prepared a My Funeral Wishes form to enable the public

to think about some of the decisions that need to be made when arranging a funeral.

This can be very overwhelming for our family and loved ones when the time comes to arrange our funeral, and can be surrounded by uncertainty if this was never discussed.

 It may be beneficial if you are able to discuss these wishes with your family/friends beforehand, and notify them of where this document will be stored. 

We hope it will alleviate some of the uncertainty for your loved ones when the time comes.

Please Note: This form only covers burial and cremation, and we appreciate there may be other alternatives widely and legally available in other countries, and in the UK in the near future.
